The first topic of the course is about a new practice approach to inpatient care. I noticed that New practice approach is all about integrating medical experience with patient care with the best evidence-based resource. Health informatics is one of the new medical practices outlined in the course. I first encountered electric health records during my way was quite an eventful day. Before the health organization introduced fully installed electronic health record, struggled to read physicians handwriting. The system makes it easier for me to access patient information on time and accurately. The use of EHR has reduced the chances of making a medical error.

A few days ago, a patient with a ganglion cyst was scheduled for surgical removal. Using the patient health record, I discovered the patient had a preexisting medical condition, diabetes precisely. I immediately informed the surgery team, who later changed their plans. I managed to educate the patient on why the surgery was rescheduled for another day. Today morning I experienced a challenge in handling patient health records. One of my clients was being released for home care; I spent almost two hours at the computer trying to retrieve the physician’s drug prescription. The system was hanging, and I had to wait for it to be back at functioning.

Interdisciplinary Collaboration

This week’s situation involved interdisciplinary collaboration within the healthcare system to improve patient care. I defined the concept as the working together of all medical providers with patients and their families to provide the highest patient care possible. The idea prevents the frequency of medical errors.

For a long time, nurses have been undermined by physicians and the hospital administration despite the crucial part they play to provide positive healthcare outcomes. I felt frustrated on Monday because some physicians in the organization still have less value for nurses. The doctor treating one of my diabetic patients seemed reluctant to answer my question regarding a patient’s treatment. The doctor had suggested that the patient be discharged from the hospital after exhibiting improved symptoms. However, the patient had requested me to stay at the facility for two more days as she awaits her daughter to fly back into the city. I asked the doctor if she could be discharged the day after, but he just looked at me and walked away. I informed the nurse in charge of the request, and to my delight, she agreed to discharge the patient on the requested day.

I managed to convince the administrative leader to hold monthly staff meetings for all physicians, nurses, pharmacists, and caregivers to develop better solutions to manage patients. Health workers are more often busy and hardly find to interact with each other. Technological systems are being used to improve interprofessional collaboration. Lack of respect for other workers reduces the level of cooperation among team members.

Population Health Issue

Coronavirus is on the rise in the united states. How population health can be improved by preventing the spread of the disease has triggered my thinking. The population health concern is to improve the collective health status of the population within my geographical location. The pandemic has adverse effects on the elderly and people with underlying condition populations.

I am becoming more concerned with the complacency level of my community in regards to mitigating the pandemic. People are not taking the safety precautions directed by the government thoughtfully.

Covid 19 mitigation can only be mitigated when the population exhibits behavior change and medicines and vaccines to be developed. I had to send back a man without a face mask who came in to visit his father, who is undergoing chemotherapy for liver cancer.

Health workers are at a high risk of getting infected with the coronavirus disease. Personal protective equipment has been issued out to health providers attending to covid 19 patients at my organization at the moment. However, the institution promised to bring more personal protective equipment (PPE) to all staff members. The question that triggered my mind was whether a behavioral change to reduce among the population is possible. Health services in the organization and the entire world have been disrupted. For almost half of my patients with diabetes, hypertension, and diabetes-related complications, treatment has been partially disrupted.

Knowledge Gained from Population Health Topic

  1. I gained analytical skills from the week’s experience. The government directives were quite challenging at first, but with time, I understood and adapted to the new laws that aimed to prevent the rise of Corona Virus disease.
  2. I had to adjust my actions concerning the patients and other health professional measures to prevent the rise and spread of Covid 19; I developed coordination skills.
  3. My leadership skills were put into use during the week; I trained and motivated patients and medical providers on ways to maintain personal hygiene and other precautions.
  4. My judgment and decision-making ability improved as I was able to examine both consequences and benefits of my actions to determine the most appropriate action to implement.

The Issue of Health Disparity

Measures to contain the Corona Virus pandemic are being developed in the whole country. However, the issue of health disparity is beginning to unfold in specific populations with other health conditions. People with higher levels of income receive better healthcare than people with low income. I realized that the poor folks are likely to succumb to acute diseases like cancer and tumors because they lack proper health insurance.

Patient X came in with symptoms of fatigue, fever, and joint pain. The man was later diagnosed with lupus condition. According to the talk I had with the patient, he had no income source for the last few years. Treatment of severe lupus is very costly to both the patients and the hospital. The patient stated that he is unable to afford a proper diet and medication. Patient X scenario is just one of the many cases faced by people with no or low income.

Health disparity, influenced by economic inequality, limits the overall gains in the quality of health care. The provision of healthcare in the country is different for people of color and Hispanic. The two races are likely to be uninsured by medical insurers due to the high poverty levels. Lack of proper insurance makes it difficult for the two groups of citizens to access health care. The issue is frustrating because I witness many patients withdraw from healthcare due to a lack of financial resources.

Knowledge Gained from the Week’s Experience

  1. I was able to offer full attention to the concerns of my patients and other healthcare providers.
  2. I developed critical thinking skills by identifying the strengths and weaknesses of a situation and finding alternative solutions to the hospitals’ problems.
  3. With the pandemic, we had to check patients coming in the facility their temperatures using improved devises, I therefore, gained some technical skills.
